CMS is accepting applications for participation in Community-based Care Transitions Program

The Centers for Medicare & Medicaid Services is accepting applications for participation in the Community-based Care Transitions Program (CCTP), according to a press release by the Centers for Medicare and Medicaid Services. 

The release states that the goals of the CCTP are to improve transitions of beneficiaries from the inpatient hospital setting to other care settings, to improve quality of care, to reduce readmissions for high risk beneficiaries, and to document measureable savings to the Medicare program. 

Hospitals with 30-day readmission rates in the top quartile in their state for heart attack, heart failure and/or pneumonia are eligible to submit an application in partnership with community-based organizations that provide care transition services.

As part of the Patient Protection and Affordable Care Act, this program will provide $500 million over five years for care transition services for Medicare patients at high-risk for readmission.
