Joint Commission announces target solution for improving patient hand-offs

Health care organizations have long struggled with the process of passing necessary and critical information about a patient from one caregiver to the next, or from one team of caregivers to another. The Joint Commission Center for Transforming Healthcare announced targeted solutions for its project on improving the quality of hand-off communications.

The targeted hand-off solutions from the Center use the acronym SHARE, which addresses the specific reasons why hands-offs are unsuccessful.  SHARE stands for:

  • Standardize critical content
  • Hardwire within your system
  • Allow opportunities to ask questions
  • Reinforce quality and measurement
  • Educate and coach

The targeted solutions for hand-off communications will be pilot tested in 2011 to examine effectiveness in demographically diverse hospitals and other care settings.
